Self-evaluation forms

This self-evaluation encourages young players critically evaluate their own abilities and identify opportunities to grow. Young players who complete this form will be more motivated to work hard because they are pointing out their own opportunities for improvement, rather than simply being told what to do. 

 

Benefit for Coaches: Players who complete this evaluation are more focused during practices and lessons.

This form helps players improve more quickly by defining goals, creating plans to achieve them, and tracking progress over time.
